D64 School Physical/Sports Physical/Vaccine Clinic

Parents/Guardians,

We are pleased to offer a physical exam/vaccination/sports physical clinic at Emerson Middle School on August 1^st.  Please remember that physical exams and vaccination requirements are due in PowerSchool by 3 pm on October 14 for all students new to District 64, incoming Jefferson Preschool or speech students, incoming kindergarten students, and incoming 6th grade students. Please make your appointment below or with another provider ASAP to ensure you have everything completed by the deadline. More information about requirements can be found on the Health Services Website.  Sports physicals are required to be completed and uploaded to PowerSchool before tryouts.
